Program Submission Form

When you schedule a YFFR class or program, submit this form as soon as possible.

Submitting and documenting information about where and when you teach YFFR is a requirement of being part of the YFFR Instructor Network. (If you are given an unexpected opportunity to teach YFFR, roll call your class on the class participation report (CPR) immediately and submit this afterward)

Notes/Reminders:

If you request an evaluation, we will send the link to you and you will share it with your participants or POC.
Evaluations on your classes or program may be sent periodically. You can request an evaluation by contacting success@yogaforfirstresponders.org
Evaluations are in place to capture great work and make any course corrections for your success
You may be asked to film your class or a YFFR representative may drop into your class
You may film your class at any point and ask for coaching by submitting the recording to success@yogaforfirstresponders.org

Class/Program Description Example: This is a 6 week program. The first class is a presentation followed by 5 weeks of classes every Wednesday. At that time they will decide whether to renew.

Describe the class(es) which needs curriculum Examples: 2-hour presentation for Fire Chiefs followed by a 30-minute demo class. 10 classes for a police academy. Class 1 is a 90-minute presentation, classes 2-10 are 1-hour yoga classes.

Would you be willing to share the payment structure for your class/program? This will help give guidance to other YFFR Instructors. Examples: I am an employee of my organization and I am paid to teach YFFR as part of my job I am charging $100/class I am volunteering for the first 6 weeks of classes until the agency renews their budget I am teaching a donation-based class in the Fire Hall
